Overview

Health Care for children extends beyond purely the medical needs of children. It includes social, behavioral, oral and family health as well. Ensuring needed services are obtained for each child and their family forms the backbone of Care Coordination. CCHAP supports the process of providing every child with intensive preventative care including guaranteeing that needed services are received by the family.

Addressing Time And Resource Constraints

CCHAP provides tools and supports for providers to develop the administrative systems necessary to support care coordination services within the practice. We interface with community resources and facilitate referral networks for practices and providers to access these resources for children and their families.
