Alternative Payment Model Updates
Description: As part of the “Colorado Department of Health Care Policy and Financing’s efforts to shift providers from volume to value, the Department, along with stakeholders, has developed a payment model to make differential fee-for-service payments based on the provider’s performance.
This payment model aims to give providers greater flexibility in care provided, reward performance, and maintain transparency and accountability in payments made. Under the proposed model, providers can earn higher reimbursement when designated as meeting specific criteria or preforming on quality metrics. Progress within this framework not only encourages higher organizational performance but also helps the Accountable Care Collaborative (ACC) achieve its respective programmatic goals.
The Colorado Health Institute (CHI) on behalf of the Department recently hosted focus groups and subsequent workgroup meetings are planned to review stakeholder feedback and refine recommendations to the Department on potential changes to the ACC APM Measure Set for Performance Year 2020.

The Colorado Access support team will be hosting APM webinars to assist PCMPs in Region 3 and Region 5 with APM measure selection to help ensure that everyone is set up for success in performance with this new payment model.

· Brief overview of APM Program goals and the measure selection process.
· Measures to consider based on alignment with other programs, EMR capabilities, and current State performance.
· Ways that Colorado Access can help support you during both measurement selection and over the 2020 measurement year.

Short Term Behavioral Health Visits in Primary Care – September 30, 2019
We have had a lot of requests for the recording, however, after listening to it ourselves we know there are parts where is it very hard to hear what John is saying. We didn’t feel that sharing the slides alone would very helpful so went through our notes and John’s slides to create a summary of the meeting (See “Transcribed Meeting Summary” in the Materials box above). This can be used in combination with the slide set in order to get the full context of John’s talk. He had some great examples that we knew wouldn’t be included if we just shared the slides. Six Short-Term Behavioral Health Visits in Primary Care…
and how to get reimbursed
Description: When the Colorado Department of Health Care Policy and Financing (HCPF) implemented the Accountable Care Collaborative Phase II (ACC Phase II), and its new Regional Accountable Entity (RAE) structure, HCPF increased access to short-term behavioral health (mental health and substance use disorder) services within the primary care setting. Health First Colorado (Colorado’s Medicaid Program) members are now able to receive short-term behavioral health services (STBHS) provided by a licensed behavioral health clinician working as part of a member’s Primary Care Medical Provider (PCMP).
The intent of this change is to provide additional access to behavioral health services for short-term episodes of care of low-acuity conditions. This may include grief and adjustment conditions, as well as medical conditions where behavioral interventions can support treatment adherence and wellness (such as obesity and diabetes). By supporting the delivery of early interventions in a convenient location for members, the Department seeks to prevent the exacerbation of both medical and behavioral conditions and treat them in the most efficient manner.
