Innovation Support Project (ISP)
Practices are to be applauded for their efforts to navigate the Medicaid APM and to prepare for value-based payment contracts. CCHAP often hears from practices that they wished there were additional resources to help support their practice transformation activities. CCHAP is excited to introduce a new opportunity for practices to receive support and coaching to ensure success with value based contracting. The Innovation Support Project (ISP) is a new initiative funded by the School of Medicine at the University of Colorado that offers practices the opportunity to develop and perfect their skills related to practice transformation. ISP is aligned very closely with the Medicaid APM so participating practices will be well positioned to succeed in their efforts.
Practices will have the opportunity to work with Practice Transformation Organizations (PTO) to provide coaching and facilitation to practices that sign up for this project. It is appropriate for all practices that accept Medicaid, regardless of where they fall along the continuum of transformation. Practices that haven’t been involved in other initiatives will benefit greatly from this one-on-one coaching. Practices that have participated in other projects like SIM, will benefit from the opportunity to continue their work where they still need assistance, including working on behavioral health integration.
From our experience, practices that have participated in other initiatives are better prepared for value based contracting, including the Medicaid APM. If you haven’t participated in previous projects, now is the time to get involved!
